Nurse Practitioner, Physician Assistant

Job Description:

• Provide approximately 45-minute in-home health evaluations
• Engage with health plan members in their homes and build trusted relationships
• Review medications and medical history
• Perform physical evaluations
• Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
• Use an iPad and clinical workflow for each evaluation
• Visit people nationwide to connect them with appropriate care
• Travel locally and within licensed states as needed
• Conduct virtual visits as needed
• Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening
• Work primarily in private residences of health plan members

Requirements:

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)
• Nurse Practitioner or Physician Assistant qualification
• Ability to travel locally and within licensed states, as needed
• Ability to conduct visits in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
• Ability to use an iPad and intuitive clinical workflow
• Anticipated weekly hours: 12
