Job Description A bit about the role The Palliative Care NP Manager will provide expert clinical care for the Palliative Care Program and provide direct supervision of palliative care clinical staff. The breakdown of people management vs patient care is likely to evolve/shift over time depending on program needs. The manager will participate in referral management, process improvement, and change management. Direct/Indirect Patient Care: Provide direct patient care to patients enrolled in our palliative care program by phone or video visit during scheduled and on-demand patient visits. Participate in and/or lead patient panel reviews Participate in weekly IDT (1h/wk) People Management: Clinical champion for the work - you are a player-coach, modeling clinical excellence for other palliativ
