Hospital Outpatient Coder II

Job Description:

• Accurately assign ICD-10-CM diagnosis codes, CPT/HCPCS procedure codes, and applicable modifiers for outpatient hospital encounters
• Code Emergency Room, Outpatient in a bed, Same day medical, and Same day surgical records
• Send queries to physicians or hospital staff to ensure proper documentation and code assignment
• Meet or exceed established quality and productivity benchmarks
• Assist in managing DNFB to minimize uncoded days
• Collaborate with Clinical Documentation Improvement, billing, and revenue integrity teams
• Identify documentation gaps and escalate when clarification is needed
• Contact providers and staff to obtain missing documentation and reduce DNFB accounts
• Abstract data with high accuracy according to established policy
• Promote a collaborative coding environment and adapt to changing department demands
• Ensure compliance with medical coding policies and guidelines
• Maintain coding credentials and current knowledge of outpatient coding changes
• Perform other duties as assigned

Requirements:

• High school diploma or equivalent
• Certified Physician coder (CPC) or Certified Outpatient Coder (COC) preferred
• Excellent organizational, interpersonal, verbal, and written communication skills
• Keen attention to detail
• Proper phone and email etiquette
• Ability to work in a fast-paced environment and meet productivity and quality standards
• Thorough knowledge of medical terminology, anatomy, and physiology
• Understanding of ICD-10-CM, CPT, and HCPCS
• Knowledge and experience using an encoder and EMR required
• Ability to work independently in a remote environment
• Ability to work under pressure
• Ability to interact effectively with other staff members
• Ability to lift 15 lbs. from floor to waist and 10 lbs. at waist level and above
• Accurate near vision, far vision, depth perception, and hearing
