Develop, assess, and facilitate complex care management activities for primarily mental and behavioral health needs
Evaluate member needs via phone or in-home visits and recommend or facilitate care plans
Conduct telephonic, digital, home, and other site-visit outreach
Develop ongoing care plans for members with high acuity
Identify providers, specialists, and community resources for mental health and substance use disorders
Coordinate among members, families or caregivers, community resources, and care provider teams
Monitor care plans, member status, treatment side effects, complications, and clinical symptoms
Facilitate care coordination and timely access to needed care and services
Collect, document, and maintain member information and care management activities for regulatory compliance
Educate members and families on procedures, treatment options, referrals, provider instructions, and healthcare benefits
Provide leadership feedback on opportunities to improve care quality and cost-effectiveness
Perform other duties as assigned

Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
2–4 years of related experience
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements, with behavioral health experience required
For NJ Health Plan: Master's degree in Behavioral Health or Social Work and Licensed Associate Counselor (LAC) or Licensed Social Worker (LSW) required
Active New Jersey license required
Must reside in Essex or Morris County, or in a nearby bordering county
Monday–Friday, 9:00 AM–5:00 PM schedule
Up to 50% local travel required
Compliance with all policies and standards
