Care Advisor, Care Concierge – Bilingual English/Spanish Preferred

Job Description:

• Assist benefit recipients with navigation of the product platform
• Recommend service offerings based on member needs and capabilities
• Support completion of orders on behalf of members
• Use assessment and screening results and community-based resource knowledge to help clients achieve care journey goals
• Perform administrative tasks supporting members with planning, organizing, and executing benefits
• Answer inbound calls and assist members with completing assessments
• Recommend service plans based on member needs and capabilities
• Make outbound follow-up calls to members
• Help members and families access resources including homecare, meals, transportation, errands, home modification, pest control, and social support services
• Coordinate and schedule services
• Meet call, quality assurance, after-call work, member satisfaction, attendance adherence, and testimonial submission metrics
• Perform other duties as assigned or necessary

Requirements:

• Passionate about improving health outcomes through home and community-based services
• Computer proficiency and knowledge of clerical functions
• Interpersonal and telephonic communication skills
• Excellent written/verbal communication skills
• Proven experience answering inbound calls
• Ability to work with multi-disciplinary teams
• Excellent time-management and organizational skills
• Ability to thrive in a fast-paced start-up environment
• Must be 18 years of age or older
• High School Diploma preferred
• Experience in Public/Community Health, or related field preferred
• Experience in Social Services, Care Coordination, Healthcare Navigation, Public Health, Community Organizing, or Volunteer Work
• Ability to remain at the designated workstation for the duration of the workday
• Constant computer, phone, and Voice over Internet Protocol (VoIP) operation
• Reliable internet connection and designated work environment conducive to professional phone calls and sensitive data
• Selected candidates may be required to undergo a background check and/or drug screen
• Bilingual English/Spanish preferred but not required
